Summary
The row containing column names is not on the first row; there are extraneous header rows, which result in errors like the following:
MBER1111 - We were unable to detect your data format nor find a header row containing the
following column headers: within the first rows. More details are available in our data import

Resolution
Delete all rows above the row containing column names
If the beginning of your file contains merged cells or irrelevant data (see the example below), delete rows 1 through 4:
Additional formatting may be required if there is information above the row containing column names.
For example, if the first account number or fund number in the file appears above the row containing column names and is not repeated in each transaction row, you have a grouped file that would first need to be ungrouped before removing extraneous header rows.
